Sign-up/Volunteer Form

If you would like to become a volunteer or sign up for an event, you may do so by filling out and submitting the information in the form below.
  • We are looking for dependable volunteers to provide Glaucoma and Visual Acuity Vision screenings. You will visit Senior Centers, Businesses, and will participate in health fairs.  Training is provided.  This is a great opportunity for you to give back to the community.
  • Swing Fore Sight golf Tournament – Monday, August 16, 2010  or People of Vision Dinner – Date TBA
